Question about Form Capabilities in Storyline

Hello,

I have never used Storyline before. My company purchased it to create training activities, although I was wondering if it is capable of creating a rather complex form.

I was wondering if I can create a form in which the user is able to add input fields in order to list items, and then new fields are automatically created based on the number of input fields the user has added. It is a little complex to explain in writing, so I have attached a number of annotated images from a dummy form I created to help explain. Please let me know if the annotations don't make sense, or if you would like more information about what I would like to achieve out of this form.

 

Can what I have described in the images be created within Storyline?
