'Rate this course' ending

Hi everyone,

Has anyone ever tried to create a 'rate this course' slide? We are thinking of creating a star rating (out of 5) and a comments field, which is easy enough to do.

What we are struggling with is how to export this information. Our initial thoughts are to create a submit button has a send email trigger attached to it, but we don't know how to export the rating and the comments.

Most of our courses are hosted on an LMS, and most of them contain assessment questions or graded scenarios.

Any ideas from the community would be greatly appreciated.
