If you are using Articulate 360, you can insert a Likert Scale survey question, and several templates will appear below the question type that you can use for course evaluations. You'll have to include a survey results slide to track the question.
If you already have a results slide, then you may try using a cumulative results slide to track both the quiz results and the survey results.
