results slide to print interactions to questions answered

Can anyone help please?

I am developing a DSE Display screen equipment course where people need to answer yes or no to questions. I have used a "survey question pick one" but I need to print the answers they have selected

I also need to add in a free text field for them to type into which I would need to print with the info and their name.

Have done all that and this is what I end up with ? I am doing something wrong, any ideas
