it does not happen in preview, but when I publish to the web. Cannot try in LMS at the moment...

can you upload your .story file

... and it is only happening with some of the question slides. Though, has nothing to do with question type...

sorry, I cannot upload the storyfile since it has confidential content... :-(

You might need to log a support ticket and get Articulate to look at your file.

I have now deleted all content slides and only left the quiz part. Please see uploaded story file. When I publish and open the HTML5 file I get the formatting error when reviewing the quiz... Maybe you can try and see if it also happens if you publish it.

I'm seeing the same behaviour using FF but it doesn't happen when viewing with Google Chrome which is the only supported browser for html5 content. - what browser are you viewing the content with?

Here is a list of supported browsers for viewing content - that might be the issue.
