Self-Eval Survey

I am trying to create a relatively short self evaluation survey. I originally started out using the Likert Scale which sort of worked, but the printed output did not break-up the results as I would have liked. So, after reviewing some of the recent posts I am trying a different way.
In all there are 5 survey questions with 3-4 sub-topics. Each topic has a 1-5 rating scale (1 = no skill 5 = role model). I am inserting individual radio buttons - but I have to figure out how to get each one to act as an "individual" button - i.e. learner can rate themselves for each question. I then need to get an output for each question evaluated which means using variables and this is where I always get stuck. I have attached my test survey with one question. I would like to be able to provide a final page with an overall eval result that the learner could print.
Would appreciate your inputs.

See attached a quick example of how to capture the selections from your questions.

to get the radio buttons to act as an individual - select them all (one group of 5) right click and select button set. What this means if the user clicks button 1 - button 1 is selected, if they change their mind and click button 2, button 1 is deselected and button 2 selected.
