Simple Quiz not resetting to initial state

I created a basic "course" in Storyline 3 that consists of three slides. Slide one has two buttons. Button 1 launches a document to be reviewed (launches in separate window). Button 2 (Acknowledgment) takes user to Slide 2 which is a graded question T/F set up as something like "Yes - I have read the document" and "No, I need to review the document." Yes (the correct answer) takes them to the Slide 3, the Results slide that says "thank you - you can now close the window." All slides and layers are set to reset to initial state. All Next/Prev buttons have been turned off. My problem is that when I answer "No" on slide 2, it takes me back to slide 1 (fine), but when I select the Acknowledgment button again and return to slide 2, "No" is already checked and it will not let me check "Yes." In addition, the Next/Prev buttons display. Published as HTML5 with Flash backup and viewed in IE11. Shouldn't reset to initial state, reset the graded question to no answer?

It sounds like you may be missing a trigger to reset the results.
With your permission, I'd like to take a look at your project file to investigate what's happening. You can share it publicly here, or send it to me privately by uploading it here. I'll delete it when I'm done troubleshooting.
