State change in question bank

I am trying to have a button appear when a user is incorrect twice on a question. I have a variable that increases and I put it on the screen to see it. When that is at 2 the button is to change states. I renamed states to avoid hidden and normal but nothing happens when the variable hits two.
When the slide is cut out of the question bank and placed in a new file the state change functions as expected.
Does something change when a slide is in a question bank?
I have attached a story file with the single question as a slide and as a question bank item. It works as a slide but not as a question.

I tried troubleshooting from a few ways, maybe this will be helpful (although not a solution)
I tried it with removing the variable condition completely - and still no state change when the layer loaded. I also tried adding a few other objects/triggers for state change to fire when timeline begins, but did not get any to successfully fire. A trigger added to adjust the variable (just trying something other than a state change) did successfully fire when the layer timeline begins.
All Object level triggers seem to work - click to change to a visited state or to a hovering state both went without a hitch once the layer is loaded. I think you are right, something is changing how this is handled in the question bank slide, and it seems something is disabling state changes for objects that are triggered for when the layer timeline begins.

It is good to know it is not just me. I have submitted a bug case so will see what comes of it. Someone has another similar post about using variables for custom feedback which could be related.
