I have created the feedback form in survey type quiz. I want to see all the questions with the respective answers and comments. Do I need to set any different settings over here. How can I get the report from LMS. Kindly help me on this.
You'll still need to include a results slide as a part of your course and then use that as the tracking method. You may also want to look at the options within your LMS to determine what information will be accessible there - and as a good testing set up you could try your course at SCORM Cloud which is an industry standard for testing SCORM content.
I have checked my module. Now everything is working fine. But "Likert scale" option is not tracked by LMS. Other type like, "Essay" kind question can be tracked by LMS. Kindly help me how track the Likert scale option in LMS. I have attached the Unit Result Property screenshot and the story file also here. Kindly help me on this.

just had a quick look at your story file and the tracking wasn't set to the Result slide it was by number of slides viewed.
I changed it to track via Results slide, published and uploaded to ScormCloud. Here are my results
