Trigger executing twice in Feedback Layer

Hi.
I have a Question Bank that each Slide has his own Correct/Incorrect feedback, on the Correct Layer it executes a trigger that Add 1 to a variable, but in some cases instead of just adding 1, it adds 2, same case when i change trigger from adding to substracting.
I already check the different between slides that do this behavior an the ones that not, and they don't have any real difference between them, not layers, not triggers.
I want to know how can i stop this behaviour because it causes some confusion to the development of the course.

Hi Wendy, sorry for not answer before, here is my .story, i only put 3 questions on the bank, but the problem is the same. in the correct layer of the 3 questions i have a trigger that is adding 1 to the variable "piezas" , but instead of just adding 1, it adds 2.

not sure why that is happening but I tend to add the trigger to change the variable 'piezas' when the user clicks the X button on the layer instead of when the timeline starts...see if that works for you - your updated file attached.
