On assessment/quiz questions due to the HTML5 issue always scoring 0% I set all the quiz slides to "When Revisiting: Automatically Decide". Which fixes this bug.
When I revisit none of the questions are reset and have all the options selected - what is causing my storyline to decide not to reset when revisiting? We have a similar course that doesn't behave in the same way.

I have an issue with the "rewind" button on the seek bar. If a user clicks the rewind button (in read only seek bar) - it seems to mess up the previous slide meaning that they cannot view the previous slide (which is set to "when revisting set to automatically decide") - would you suggest setting all slides to "resume saved state"?
Are they unable to return to the previous slide or it's just not resetting the slide? If you'd like it to be reset, you'll want to use "reset to initial state" and if you'd like it to pick up where you left off, you'll want to use "resume saved state". Also, here is how the rewind button will behave based on your slide properties.

I never use automatically decide only reset to initial or remember saved state, for quizzes I use remember saved state

remember saved state works best with questions if you are reviewing them
Sure, but its an assessment and ideally they should be reset when they retry the assessment rather than have their options saved or remembered
