The way we manage the change request (eg, content change) is very messy at the moment, reviewer sent in the content in multiple formats (bullet points, tables etc), which make it harder for the developer to follow.
I am looking for a 'Change Request' form that anyone is happy to share?
Not sure if this is what you are looking for, but Christy Tucker has a spreadsheet that is incredibly helpful for tracking changes. If you are using Google you can send it out and have the SME's make the changes, and then mark when the developer has made the change:
Here is one that I use. I am not sure where I got it so can't take credit for it but it is good for getting agreement up front before changes are made.
