How to add a phone number into a software simulation

You could use a Freeform Text Entry question or a Fill-In-the Blank question. Another option would be to add a data entry box to you slide and use triggers to display correct or Incorrect feedback. See screenshot below shows a Freeform Text Entry.

Thanks Michael - this helps.  I think the data entry box is the way I will go because there are several boxes that the learner will need to fill in on the screen.  I am able to set-up correct and incorrect triggers when the data is text only.  I am struggling with creating triggers for the phone number data entry boxes.

At a glance, I don't see anything wrong with that trigger setup. You could simply copy&paste that trigger for the one that shows the incorrect feedback and adjust the conditions to say 'is not equal to'.

Wouldn't you know it....I worked on this a ton yesterday and it would not take me to the Correct or Incorrect layers.  However, as soon as I post this question to the community it all works like butter....oh brother :)
