Open response questions with print option - Java script expert help needed!

I am attempting to have five open response questions on Storyline where the last page allows learners to print their responses to all five questions. I used a template that was shared here, but the post indicates the need to tweak the java script to add additional slides. Currently, it is set to only print the response of the last question. Are there any java experts that would be willing to take a look?
