Self-Assessment with varied points for each response

I'm trying to develop a self assessment in Articulate where participants rate themselves between 1 and 5 for each of a series of questions, and the results tally the total numeric score (sum, not %). I've tried searching for templates or instructions a number of ways to no avail, but I suspect it's my search terms. Des anybody have and information or resource links that might help? Thanks very much!
