Workflow Process - how to form

I'm working on a workflow process for a complicated request to get a password for a gov't agency. (Articulate 360) I would like to create a form in Storyline 360 to replicate the workflow.  Does anyone have any suggestions for having learners enter the information and receive feedback for each section?

There is no LMS. I believe this will be a standalone module
