When you publish to Articulate 360, you have the option to Create a new item or Publish a new version of an existing item. Do you think it's possible that you published a new version of a lesson you had previously published?
Hey, Travis! That's an excellent feature idea, and I'll be glad to pass it along to my team for you.
For now, you'll want to republish the latest version of your project using Create a New Item. This will generate a new URL and disassociate all errant versions.

Thank you; it is possible that I published a new version. Can I undo or delete the new version?
Hey Travis! You can go back to view a previous version by clicking the Current Version dropdown. Do you see any previous versions listed there?

I can see the previous versions. I need to delete the errant versions that I added.
