Typically you would see this when you previously unzipped the files for a SCORM package, and then unzip the same files (e.g. an updated version of your course) to the same location for a second time.
That's certainly strange – I can't say I've seen this happen before! Try deleting all the old files on your hard drive that relate to this Rise 360 course, then republish the course for LMS to get a new output file.
Right-click the output file to unzip it, and let us know if you run into this problem again! If you do, it would be helpful if you could take a fullscreen screen recording so we can get a better look at what's happening on your side.

Nope, it is happening on new never before opened files.
