Have you tried the Preso app for the iPad? Preso is part of the Articulate 360 suite. According to the details it is designed to create video training on the iPad which can be added to a Rise or Storyline project.
You may have to use screenshots of the mobile app.
As for recording what is happening on your mobile device there isn't an Articulate tool that would do that, but I bet the community have a lot of suggestions so I hope they're able to share here!
Depending on the OS of the mobile device, there are a number of tools available that mirror the device screen to a Windows desktop, which can then be recorded. One of these tools is Reflector. Specifically for Samsung Android devices, you can use SideSync. Here is an example of an Android device screen, mirrored with SideSync and recorded with Storyline.
