My client would like to have a form embedded at the end of the course to allow the student to fill out and print. Any ideas how to do this? Is it possible?
You could also look at creating a fillable form using a third-party tool (like Google Forms). You could embed that form in your lesson using a multimedia embed block.
Next, host the PDF on a web server so you can get a sharable link to the PDF. You can put the link into a multimedia embed block to embed the PDF right in your Rise lesson.

There is no block in Rise that could do this.
You could create a fillable PDF form in Adobe, and then upload it into Rise.

I just tried but the PDF looses its fillable capability. Any tips of where I should be hosting it or something?
