How do screen readers process image blocks with captions?

I've been using a full width image block with a background image to add visual interest between blocks. Then, in the caption section I'll put the title for the next header.
See attached image for an example. The Alt text for these images is the same as the caption

For screen readers, how will this display? Visually, it makes sense. But, will the screen reader read the Alt text and the caption (the same text twice?)?  Any downsides from this?
I know using Headers vs increased text size is important in web design. Is using a Header block more accessible than the current method of titles in a full text image caption?
