LMS Report

I exported my Rise training as scorm 1.2 to upload into our LMS system. As our employees complete the course in LMS is does not report accurately. The course has an assessment at the end which is the last lesson. The user must pass with a 80% and they have 2 attempts to pass. Employees state that they have passed the assessment but the reporting is not reflecting this. Please help. What could be the issue?

I am having the same problem. I have the exact same settings -- 80% required to pass, two attempts. But after failing twice, I am still able to try again (even though my settings are selected to limit this). So when I continue on and exit the course, I am being marked as "in progress" instead of incomplete?
b/c the LMS reporting is not working properly we asked everyone to provide screenshots of the course completion and score. Based on those screenshots 25 employees completed the course. 9 failed and 16 passed.
The status report in LMS is attached. It only provides the status of 10 employees which all are listed as passed. This report is inconsistent and does not match the information I was able to obtain from the employees. Not sure why this is.
