How to create a survey

What is the best way to create a survey?  Is quiz maker the only way?  At the end of the "survey" it is giving a graded score which makes it feel like a quiz vs. a survey.

Hi Megan,

You can definitely make a survey in Quizmaker! Here's how:

  • In Question List View, go to the Home tab, and click Survey Question.
  • When the Insert Questions window opens, select the Survey tab in the upper left corner, and choose one of the question types. When you click a thumbnail, the right side of the window displays a description of that question type.
  • Click Insert.

For the results slide, change the Result Type to Survey. The survey result slide simply tracks whether or not the learner completed your survey.

How do you have the tracking set during publish? I know the default for a Survey Result slide is typically Passed/Incomplete.

Are you able to replicate the issue you are seeing in SCORM Cloud or is it limited to your LMS? If you need us to take a look, you are welcome to share the .quiz file.

Hi Katie!  Thanks for sending along your file.

For question and answer choice text, you'll want to publish for SCORM 2004.  I published for SCORM 2004 and uploaded your course to SCORM Cloud (SC) to see how the answers reported.  I kept getting "undefined" as the learner's choice.

I realized because there was a space between the "Yes"/"No" text and the radio buttons, that SC couldn't read the answer text.  I removed the space in front of Yes and No, and SC was able to report that the student answered "Yes" or "No."

The takeaway for you is:

  1. Remove the space between the answer and the radio buttons.  I realize it might be a little tight-looking, so I'm investigating why the space is causing issues.
  2. Publish for SCORM 2004 if your LMS supports that version.

This update fixes the issue you reported where non-English characters and symbols would cause quiz data to be undefined in LMS logs and reports.
