It may be an issue with how you're testing the course. Are you viewing the quiz output files locally when you test this by any chance?
If you view a published Studio '09 presentation on your local hard drive or send it to someone else to view on their local hard drive, you'll encounter security restrictions from the computer, web browser, Flash Player, and network that'll cause various features of your content to fail.
To properly test your published content and share it with others, upload it to the environment for which it was published. Please review the article below for more information:

Just wanted to follow up as I saw Karla sent you a solution on 7/29 to ensure you had the most updated versions of Studio and Flash player installed, in addition to disabling pop ups within your browser. If you are uploading to a host, are you using a web server or LMS? Also, which web browser are you using to test the project?

Could there be any connection to Win8 + ie 10 on the PC I'm testing the product?
