Publishing a Quizmaker Certificate

Hello,

I am using Articulate Quizmaker for a specific course and at the end of my course my client wants to have a certificate he created in Microsoft Publisher be printed out for the learner's records. 

How do I add that specific document into Quizmaker? Also, there are specific fields he wants to be filled out, including the name, the date, the person's supervisor, and the facilitator.

Would I just introduce it as a new slide with text fields they have to individually fill out?

I am attaching a copy of the certificate as reference.
