2 Issues with SL2 MR Quiz - Reporting Results and Feedback Management

I've got 2 issues with a "course."  I'm using SL2 v6 Multiple Response.

Issue 1 - Reporting Quiz Results
I'm trying to use a Multiple Response (MR) Quiz to serve as a means to record acknowledgement of a completed task. It is a one question graded MR with 3 choices. The "correct" response is to select all 3 checkboxes which submits the results and advances to user to the results slide. My problem is that a "passed" result is not registering in the LMS. Can someone look at the attached example and tell me what I'm doing wrong?

Issue 2 - Feedback Management
I've configured this 1-item quiz for multiple attempts. There is no Correct feedback, but it does generate Try Again. I've got it set up so that a Incorrect/Invalid response and the Try Again feedback look identical as Incomplete. Problem is that they aren't.
Incorrect - The default Incorrect displays the Incomplete header left justified, when I want it to be centered.
Try Again - the Prevent user from clicking on the base layer property won't take.

Can anyone help me with either if my issues? The sample is attached.

Hi Paul!

Thanks for sharing your file and such a detailed description of your issue.

Issue 1 - the course was marked to track by passing the quiz with 100%, but even when the user selected the correct response - there were no points associated with the correct answer, therefore the score was 0% and the user was failing.

Issue 2 - I'm not sure that I'm seeing the issue reported. I only get the 'OK' feedback which is tied to your Try Again it seems. With unlimited attempts, you would never seen an incorrect layer if it were there.

I added a 1 point value to your question slide when correct and deleted the Incomplete layer.

Thanks Leslie. I realized the point issue after a good night's sleep. I made the same fix you did and verified by having results display n the results slide. All good.

As for the feedback, when the user submits with no checkboxes selected, they get auto-generated Invalid/Incorrect feedback. If they submit having checked only 1 or 2 but not all 3 check boxes, they get the Try Again. (Set aside the logic of why I would want/need both feedbacks and focus on the display/functionality concerns.)

Invalid/Incorrect displays the feedback header to the left ( don't want) also, the base layer is inactive (I do want). 

Try Again displays my custom formatting for the header, but in spit of activating the Prevent user from clicking on base layer property, the base layer is active. I added a Welcome slide so that the base layer would have navigational interactions to demonstrate this.

So, how do I get the text formatting on the one to stick, and how do I get the Prevent user from clicking on base layer property to stick on the other? I've tried the Feedback Master.

Thanks for the additional details Paul. I misunderstood you originally and did not think about the blank submission. This is built in, but I invite you to check out this similar forum thread here with some workarounds that you may want to consider and may assist you.
