One of our SMEs indicated that some of the questions in the Storyline he was reviewing had the answers selected automatically when the questions were loaded. The way that we set this up was that there was a Knowledge check in the Storyline which draws questions randomly from the Question Bank. We had 17 questions in the Question Bank. The thing that really puzzled us was that he was experiencing this without any certain pattern. It meant that it was not on certain questions, or certain sequences. Any suggestions or helps are welcome. This one really stumped us.
You mentioned not certain questions - but question types? I can't say I've ever seen a radio button or checkbox automatically selected upon visiting the slide - but it's possible that a sequence drag and drop for example is in the correct sequence upon visiting just based on the random nature of shuffling. Were you able to replicate this behavior? Where was he viewing the published output? You'll want to make sure it's been viewed in the intended environment and within one of the supported browsers here.
Hi, Ben -- That certainly does sound odd; sorry to hear your SME is experiencing these difficulties! Can you confirm that he is working locally as described here, and he may want to try importing the file into a new file. If you are still having issues, you might want to try to repair Storyline to address this type of erratic behavior.
Also, could you please also check that he is testing the published output in the intended environment?
Thank you for your responses. We uploaded the published course to ReviewMyElearning.com. Just to clarify it was a published course that the SME was reviewing not the Storyline file. My team were unable to replicate this issue, and so far he is the only one with this issue.
Thanks Ben for the update and additional information. If it was just one user who saw it, let's hope it was a fluke or that he clicked on something without noticing it. Please feel free to keep us posted if you or others see the issue again.

We finally figured out what was happening. We had the SME clear his IE cache, and now everything works fine. Thank you for all the responses.
Glad to hear all is well, Ben, and we appreciate the update. :)
