I know of this example where it's multiple fill in the blanks, and here is an example with a few true/false questions on one slide.

Hi Ahmed,
Storyline by default only allows for one question per page - but there have been some examples where users set up something similar. I know of this example where it's multiple fill in the blanks, and here is an example with a few true/false questions on one slide.
