Glitch? Portion of course appears in feedback layer

We currently have several thousand customers accessing one of our courses that was published in full HTML5 SCORM 2004 from Storyline 360 and is running in SCORM Cloud. We have seen several instances from customers where a portion of a specific slide becomes embedded in the frame a feedback layer of a question. In other words, after a learner clicks submit for the interaction, the feedback layer pops up. Only instead of having the programmed applicable feedback in it, it's acting almost like an iframe for another completely unrelated slide resulting in a picture in picture type of effect.

This does not happen to the majority of our customers. Nor does the erratic behavior occur on the same slide for those who experience the problem.

Is there a known cause for this behavior? Is there a solution?

More info on the issue:
While troubleshooting to figure out what is going on, we used the Dev Tools to manipulate the feedback dialog Div via css changes, and determined 1) the 'instance' of the course that appears inside the FB dialog is controllable from the Player contols; 2) no data communication between the instance and the LMS occurs, which makes sense; and 3) the original 'Parent' course is completely inactive.
