How do I create a summary page that pulls the learner's unique answers in text fields?

Hello,

I'm really hoping you can help me. I'm creating an Unconscious Bias eLearning module where I would like to be able to create or import a slide that will pull the reflection questions and the Learners unique answers at the end of the module. Kind of like a summary. This way they can print/download their answers for review with  managers.
