Issue with Multiple Response Option

I'm encountering an issue regarding the multiple response option. I have a question with four choices where two are correct and I've selected the multiple response option. However, during playback, even when choosing the correct options, it consistently prompts an 'incorrect' message with unlimited attempts or 'You did not select the correct response' with a attempts limit set to 1.
I've verified the correct options are marked in the question settings. Any insights or suggestions would be greatly appreciated.

Thank you for sharing the screenshots!
What I noticed was that your correct answers are "It prevents glare" and "It is placed at eye level", which are at the bottom of the list, in Form View (Screenshot 2). However, since you also selected the option to shuffle, they are no longer in the same position when previewing the quiz (Screenshot 3). It looks like "It is placed at eye level" is the second answer from the top, and was not selected during that attempt.
