If i understood this correctly, I have experienced a similar problem before. I had an image as part of a hover state, but it failed to appear after the first instance of hover.
I worked around this by using a hotspot to create the hover effect. (Change state of... when mouse hovered over...).
Feel free to try this otherwise upload a story file and someone will have a solution i am sure of it.
If I'm getting this right, why this happens at times is because the text isn't properly pasted on each of the states. Try copying the text from the Normal state, going to EDIT STATES, and pasting it on the Selected state manually. Hope this helps!

Can you upload your story file - might be easier for someone to help if they can see your file setup.

I will upload a file shortly. This happens anytime a state is used.
The create new state or duplicate state options make no difference.
