LMS Not Reporting/Returning Question Level Data

Here's the skinny... We use a fairly popular LMS in the financial industry. Within the LMS, they deliver pre-built courses for our employees but we also want to upload custom courses with SCORM files.
There have been attempts to get question-level data with test courses but we feel like there may be some sort of switch that we're not flicking or settings that are not engaged correctly because learner responses are not being returned within the LMS. We know that the system can return learner data because the pre-built courses offered by the vendor does generate reports for us.
Can someone take a look at the attached screenshots and offer guidance. Perhaps there is something claringly missed?

Thank you for reaching out and sharing what you are experiencing with your course published to your LMS.
It looks like you are only hoping to report on Slide 1.5. Is that correct?
Have you tested in another LMS environment, such as SCORM Cloud? This will help confirm if the issue is specific to your LMS or the published project.
