Looking for a way to report on 'missed questions' in a quiz

I'd like to know, for any given quiz, how often each question was answered incorrectly. The preferred count method would set the 'miss' count once for each user; the count latches the first time they miss the question. If we have to count all misses, that's OK too but not preferred.

We want to have a way to see where multiple users are missing the same question at an unusually high frequency. This allows us to find bad questions or bad source material used by users when taking a quiz.

We publish to SAP's LMS and, I believe, all reporting is derived from the LMS.

I'm quite new to both StoryLine and the LMS so please feel free to point me to any available resources. I did a search before posting and found the link above.
