Magazine style survey

Hi Heroes
I'm trying to create a survey which provides learners with a result describing a personality type, based on questions answered. Now I have looked at all the discussions related to this topic and I could not find a solution to my question.
I have four words per question. The learner must allocate a number to each word (4 being most resemble with, 3, 2 and then 1 the least resemble with). Each word/option should have a score allocated which should tally in the end and should then give the learner the personality type (similar to DISC and VARK).
I've also watched the tutorials and videos and cannot find something similar to this.
Can this be done in storyline? I'm hoping someone can help please? :)
