My client would like to have students enter their name at the beginning of the course, and, if they pass, be able to print out a certificate with their name and date on it. A caveat is that this will be stand alone on disc, not tied to an LMS. I have read thru a few threads that all seem to use an LMS.
Although we don't support certificates within Storyline, there is a method described here on how to make a certificate. There is also another thread here with a lot of different ideas or examples on how to create a certificate.

For the first, the author uses the report.html file, however I do not see that in my published material. Is that changed from when he did this tutorial in 2009? If so, what steps might I take now?

The report.html file is generated as a part of the "print results" feature added to a results slide. The screencast is technically for Quizmaker 09, but within Storyline you'll find the report.html file in the story_content folder of your published output.
I looked in that folder and there is no report.html. Why would that be?
Also, on another topic, I want the students to be able to retake the assessment if they desire, however when I return them to the assessment, I see the same questions with the answers they already input. Is there a way to reset the assessment questions to their initial state?
Did you enable the "print results" feature for your results slide?
As far as retaking the assessment, you could use the "retry" feature of the results slide to allow the user to go back to the beginning of the quiz questions and have them be reset. This tutorial walks you through all the elements and feature of the results slide and how to set them up.
