Surveys & Scraping Data

Hello.  We often have a link to a survey at the end of our courses.  However, they are too frequently filled out in ways that make it hard to identify the course they are commenting on, despite clear instructions in the survey to name the course.

What I'm wonder is if there's a way to create a form inside of the SL project that would stamp the course name at the top and send us their comment text...outside of an LMS (these course are not hosted by us and often in other LMSs, so it's not as simple as sending it to an LMS).  Is there some way for an articulate file to place information into a sheet or database outside of their LMS?
