Question Bank - randomization options

Newbie here! I have two questions about randomizing questions using a question bank.

1. When all of the questions in your question bank are to be randomized and all of them are included in the quiz, do the questions appear in the order in which they appear the first time through? (The questions are then randomized on retakes.) If no, is there a way to do this?
2. How do you randomize the answer options for each question? Similar to the question above, we want the options to appear in a specific order the first time through, but then randomize on retakes.
