Question banks may not contain draw from blank slides: where blank

I've got a couple banks of questions.  When I hit "New Slide" and "Draw from Bank"
I get the error message "Question banks may not contain draw from blank slides."

I went back into the bank and there were no empty slides. Not sure how to locate the
"blank slide"  I've got a bank of Multiple choice with that error message and a bank

of Fill-in the blanks and get the same message.  I'm stumped and stuck. How do I

correct the error it is observing.
