quiz feedback layer elements not on my master

Whenever I create a new quiz, it seems to be bringing in a default feedback layer that doesn't show up in my feedback masters.

It has a feedback message and a "continue"  button in a different location than on any of my feedback masters. Changing to the desired the layout for the feedback layer brings in my correct feedback master, but the incorrectly-located text box and  "continue" button are still there too. I have to manually delete these elements for every feedback layer.

I can't find anyplace to edit these default elements. I did not create this original file. Did the creator "hide" some defaults in here somewhere?

Hi, Donna -- Many thanks again for your patience while I reviewed your files. I took a look at your Feedback Masters and also did to see where the default feedback layer information is so that it could easily be removed, so I went ahead and created a support ticket on your behalf for further investigation. One of our engineers will be in touch once they have had the opportunity to review your file and I plan to follow along with their progress. :)

UPDATE: I wanted to stop back in to share the resolution that was reached by our support staff who assisted Donna, as hopefully it will be helpful to others who may come across this thread with a similar issue:

It looks like the project was created using a Storyline template and then the author removed the design for the feedback layers or it was corrupted. I can't find the design of the feedback layers from the Feedback Master also. 

If you need to get rid of the default design for the feedback layers, I suggest importing the project in a new Storyline project. Doing this will remove the default feedback designs. Just make sure to save a backup copy of the original project in case something happens.

You can import from a Storyline template (image from your attached file).  What you could do is to start a new project and then click File > Import > Storyline.
