Quiz Question

Good afternoon -
A seemingly popular trend with our organization right now is to ask quiz questions that look something like the following image:
Currently, with Quizmaker, we came up with a work around of using a Survey question using a likert scale, and just have people click the radio button for the answer. This isn't perfect, as we are being requested to allow two attempts with different feedback between the first and second attempts if they get it wrong. Using the survey question, we just give feedback as something like : Check your answers - and give them all the correct answers.
Does anyone have a suggestion of how to make a question like this work? We suggested that they use one question per slide, however, that has not gone over well either...

A freeform question can be included in quiz results.

Gerry - these are great! Exactly what we were looking for. Once we start producing in Storyline, we will make sure to use this capability!
