Quiz results incorrect

Hi
I have created an on-line assessment for our occupational health department. The assessment uses questions to ascertain if the candidate requires further attention. The maths surrounding this are a bit complicated but in essence, a result of 81% or more is a pass. I get a report from our LMS which provides both the score and the result (percentage + pass/fail) I've noticed a number of candidates are scoring 79%, some of whom achieve a pass and others a fail. As the pass mark is 81% I don't understand how this is happening. Any ideas?

If you're experiencing issues with LMS I'd begin by reviewing the following article, which outlines common issues pertaining to viewing and tracking content in an LMS. Specifically, I'd look at testing it within SCORM Cloud, to see if the behavior is reproducible there as it's an industry standard testing site for SCORM Content.
If the issue persists and isn't reproducible in SCORM Cloud, open a support case with your LMS provider, since they're your primary source of support for LMS tracking and reporting issues.
