Review Quiz showing "incorrect" even when answered correctly

I have an 18-question quiz ending with a results slide with the Review Quiz button.  When I click it to review the quiz, the "incorrect" banner appears across the bottom of every questions--even the ones I got right.  Any ideas as to why?

Update:  Turned out it was a simple problem. Question slides were set to "Reset to Initial State."  That means they reset during the review process.  Since the questions were now blank, they were all considered wrong.  Needed to set them to "Automatically decide."   Thanks to Articulate Support for figuring it out.
