Also, what LMS are you using? As it sounds like a LMS-related issue, I would encourage you to reach out to their support team. Let us know what you find out!
What Learning Management System (LMS) do you host your courses in - for example, Blackboard, Moodle, Oracle, etc.? Have you reached out to your LMS team to see if they could help track down the issue with scoring?
It's up to the LMS if they'll keep the one original score, or overwrite it if the user attempted the course again. I'd ask your LMS team to weigh in here!
We are using Moodle. Already I've confirmed with the LMS team and it's scripted to retain only the original score; and it has gone on well with all the other users.
It's only in these 2 cases, that the scores were reset to zero. Hope you would have seen the sample screenshot of the log entry.
Could there be any other possibility for this bug? Pls guide us.
Thanks for that info - I'm sorry it didn't reveal anything else to look into! Was this user able to update their score by reattempting the course? Or were you able to manually reset the score?
Did you LMS team spot anything in their logs that would indicate an issue with completion?

If it's only happening to one or two users out of a larger group, I'd look to specifics of those users, i.e.:
That would help narrow down when it's occurring!

Using Chrome (v.58) and Firefox (v.56) latest versions.
Exit from the course on clicking the Exit button within the SCORM package.
Leave from the course when they receive certificate, which is the last screen of the module.
