Setting up a Likert Scale survey that provides feedback to participant

Hello Everyone,

I have a quiz that currently runs offline which I need to put online. There are 15 questions and the user currently manually inserts their answers from the quiz into another table which then adds up your score to equate to a rating against five different domains/areas.

As there are no right or wrong answers I had presumed this should be a survey, so I have tried putting it into five groups, each as a Likert Scale survey on a rating scale of 0 to 7 (possibly sliders), however, I cannot find any way to rate or get results out of this in one combined way. Ideally, my idea was to have the results combined into a radar format. (see attached)

I have included an example of the questions and the radar format I was hoping to have the results somehow appear.

I have been trying to develop this for a couple weeks with no success. Can anyone help please!

Hi Ashley,

I haven't received any ideas on my post. Is there a staff person who might be able to give me some insight. I need to have this project done by the end of the week. I would really hate to have to resort back to the paper version, when the entire course has now been built on Rise.

Hi Ann,

Are you looking to build this in Storyline or Rise? 

As Phil mentioned, it may be something you could build using Javascript, and that's something I'd have to rely on the community's expertise for, as it's outside my knowledge! Let us know what tool you're authoring the courses in, and then folks in the community can point you in the right direction.

Not sure anyone can help, but here is what I started on Storyline. I just don't know how to tabulate it to at least get a score for each grouping of 3 questions (in 5 areas). Ideally I would love to have all the ratings come together at the end and then find a home in a radar format. But I am desperate so would take even just a score for each area and then have participants plot their own score on the radar.

Thanks Russell for the great radar chart. I am also looking for creating one of those charts. I just have a question, after looking at the triggers on the file, I could not now how to link the results from the slide to the chart.Js . Or this work needs someone knowledgeable with javascript work?

I am thinking of using other chart type that's why I am thinking how to link the survey results with the chart.
