Storyline 360 scorm package not launching

Hi,

I'm having trouble with a scorm package not launching.  It is a series of 5 modules (scorm packages) within one course.  When I uploaded it a month ago it launched beautifully, the other 4 modules all launch well, this one just decided to stop today!

I tried re-publishing and zip the Storyline course again and re-upload to our LMS, but no luck.

I've attached a copy of the screenshot to see what I'm getting, and a copy of the file.

Any help would be very appreciated.
