Storyline assessment issues

We are have issues with a Storyline assessment. We are getting numerous complaints from employees taking this course that the assessment prefills all answers (with wrong answers), and they cannot change the answers. It will allow them to submit, which results in a failing score. Cleared cache and restarted computer but getting same results. Another employee tested the assessment, and it's worked fine for her.

This course has a test-out option. My peer and I are troubleshooting. We both intentionally failed pre-test and then got to end of course. I passed the final assessment, but she answered a few questions on final assessment, and then the remaining answers were prefilled and failed her. The only difference is that in the pre-test, I didn't review my answers before continuing, and she did review. 

A month ago this same assessment glitch happened to me but with a different Storyline course that did not have a pre-test options. Our HR group told me last month to change browser. I don't recollect that changing browsers fixed the issue, but it did eventually work.

My co-worker thinks it's a Storyline issue. We are comparing triggers with other courses with assessments and pre-tests.

Can anyone help with this dilemma?

Your problem does not sound like a browser or a cache problem

 i need some more information

  • which LMS is used?
  • Scorm 1.2, Scorm 2004, xAPI, ... ?
  • is the Assessment a separate .story file?
  • what are the settings for resume - "Prompt to resume", "Always resume", "Never resume" ?
  • does the problem with the pre-filled answers occur at the first try of the assessment?
