Here is the problematic slide in the attachment. When the answer is incorrect, I need to submit the answer several times before it is accepted and I can not figure it out why.
You have 4 attempts, but sice there is no Try again layer you see nothing on these clicks. You can restore the Try Again layer by adding text in the Try again field in the Form view.
