submit registration info to/via email

Hi,

I am trying to create a registration slide for staff induction module in which the user is required to fill in name, email and ID and submit to an email address or directly to some sort of a record to keep a track of who’s completed the L&D training module.

Any ideas of how to do this? Many thanks
