Test with different values for each answer

Hello Heroes,
I try to tackle the test you see in the attached picture in the best way possible.
It is a self-assesment with 29 Questions in total. Every answer adds a value from 0 to 3 depending on the answer you give (like true for you, sometimes, never, etc.) for a total score, that gives three different results depending on your final score.
Now I know how to work with variables, but the execution here gives me some headache. I don´t find a fitting template for this sort of self-assesment and by building it from scratch with boxes you can tick (and then add +xy to score) I struggle with limitations like clicking and unclicking a box multiple times (each time adding a value).
What would be the best way to approach this test in Storyline360?
